1) Fill out your contact info - name, email, and/or phone number.

2) Choose Selections to add to your order. Your selections will appear in the Order Summary below.

3) Input the quantity for each item in your Order Summary.

4) Choose a Store for your Order Pick-up.

5) Select a preferred Pick-up Date. If you have a comment or special request about your order, you can mark it in the Comment space. (Optional)

6) Once you are finished, click "Submit Order." If you need to make changes to your order, click "Reset Order Form."
